Correspondence with the nonprofit organization that was generating these transactions revealed that they were continuing to charge a card which had become invalid. Their method of handling cards becoming invalid is to ask the donor to initiate a new scheduled donation using a valid card, neglecting to stop charges to the old card.
So "Card Replaced" means that the transaction was an attempt to charge a card that was no longer valid. It's true that the invalid card had been replaced, but that's not the essential information about the transaction. What it should say is "Declined - Card No Longer Valid".
