i have not been able to use spoken text for months since getting my iphone 14 pro max and doing the update that occurred at that time in october. The pull down from the top with two fingers does not work unless you're in landscape mode. and it skips words constantly. (always the same words in any given text selection)
Changing the settings in spoken text doesn't seem to matter with regard to the missing/skipped words.

I don't know whether this suggestion will help, or not.
With VoiceOver,
Your iPhone uses a compact voice for VoiceOver, Speak Screen, and Speak Selection.
And you have a few other options for the voices that you can use for speech.
Seems you can also switch between Default and Enhanced Quality
