Will Apple Care replace my stolen iPhone?
already activated the “lost mode” for my stolen iPhone. With my apple care, am I able to just get a new phone?

If your AppleCare+ package specifically covered theft, file a claim. Otherwise, check any non-Apple insurance policies which might cover it and buy one at the regular price.
