Since ios16 my alarm only plays through my earphones ? Where as it used to play through my phone even if earphones were connected, as someone who falls asleep with earphones in but they inevitably fall out during the night this is a huge issue as I am not hearing my alarm… can I fix this ?

I have this issue too. I cannot believe Apple is not fixing this ASAP.
Messing with code that involves critical functions should have been tested and eliminated before release. There really is no excuse for this.
