My iPhone 13 Pro updated to iOS 16 and then my watch automatically updated. All data on Fitness app seems to have disappeared as it asks for entry of height etc. I cannot see the Fitness (Activity) app in settings either. Have I lost all my badges and awards??

Unpair the watch and when you re-pair restore to a recent backup that you know has all your awards and badges.
Don't restore from the backup made when you unpair the watch because that backup will have none of your previous data and awards you need to select an earlier one from the cloud.

Try a forced restart of phone and watch.
