I have updated to iOS 16. I set a photo as wallpaper but it only appears on my Home Screen. The Lock Screen photo is very dark and out of focus. Previous versions showed the photo clearly on both home and lock screens.
Has anyone else got this problem?

Well after leaving my iPhone 12 Pro Max alone for a little bit the photo suddenly appeared on my Lock Screen. Don’t know why it didn’t show when it was set to begin with. But seems to be working ok now.
In the pic you posted, the "Do Not Disturb" focus is on. By default, Do Not Disturb will dim the lock screen. You can change this in the settings for the Do Not Disturb focus. Go to "Options" in those settings, and there is a "Dim Lock Screen" setting.
Not for me, I don’t have the option.
I can see this is an option in DND, but it isn’t for “sleep”. So I’m still getting the same problem as OP :(
The "Dim Lock Screen" option is available in every focus mode, so maybe it's enabled in your current focus mode?
Apparently you can’t turn it off on your Sleep focus. 🤷♂️
