what are the requirements for the AirPods Pro replacement?

You need to contact Apple Support and troubleshoot your issue with them.
Personally, I used the online tool to replace my AirPod that I had issues with.
I received a replacement, shipped my old one back. If my AirPod was tested and no issues were found, I would have been charged for the replacement.
