In certain countries you can extend Apple Care+ to a monthly payment version within 30 days of expiry?

The link you posted from, and WheelieNick provided as well explains how to extend your Apple Care+ plan. What exactly confuses you from that?

Purchase the new coverage within 30 days after your original coverage ends. To find out if you can purchase the new coverage, go to mysupport.apple.com and follow the onscreen instructions. The new coverage automatically renews until canceled.

I don’t understand what you mean I looked at this page and am still confused

Things seem pretty clear. Please review again a bit more carefully.

If you paid upfront for an AppleCare+ plan for your iPhone, iPad, or Apple Watch, you might be able to purchase new coverage that renews monthly.

Seems pretty clear. You might be able to purchase new coverage that renews each month.

Again, pretty clear. You have to purchase new coverage within 30 days after the original AppleCare coverage ends on your phone.

To find out if you are eligible to purchase new coverage, you need to go to this website and follow the instructions:

Your plan coverage may continue beyond 24 or 36 months, on a monthly or annual basis, until canceled. Pretty clear as well. If you qualify for the new coverage, you will need to cancel it at some point when you no longer need the coverage.
