Hard resetting failed

Hello, what can I do to my Mac, it keeps showing a flashing question marked folder when booting, even when I hard reset it. No command Really works on it except (Power button and opt +R )

What do you think has happened to it please?

Sounds like your erase and reinstall OS operation ("hard reset"? ) failed. If your Mac starts up to a question mark - Apple Support (IN)
