Back up is very confusing

Should I delete back up on my phone? Would it delete the everything or would it be fine and I can back it up again?

I need to know since my iCloud is 90% full of backup stuff on my phone. I don’t know if I should delete it and if it would affect anything? Please help!

Deleting a backup means you will lose everything, and if you will want to restore at a later stage or on a new phone-you won't be able to. Unless, if you don't need this particular backup anymore and it is of a very old device not in use by you any more.

The best option you have is to purchase iCloud storage and leave the backup as it is:
