How do you update iPad with out Wi-Fi?

You could visit an Apple Store, or try updating it using Finder (on a Mac), or iTunes on a PC.

An iPad requires a WiFi hotspot to connect to the Internet.

You have access to the Internet, which is how you are communicating with us.

If that's not possible, then you can take it to an Apple retail store and use their free WiFi hotspot.
