Can apple watch 6 have an alert go off if my heartrate drops below say 120 bpm?

If you're asking if you can set a notification so that when you're exercising, you'll be alerted if your heartrate drops below 120 bpm, no, you can't.

You may be able to find a third-party app that will do that but I've never looked for anything like that so can't say for sure.
