How can I prevent my child from re-downloading an app that was once deleted?

I once approved the purchase of an app on my son's phone. I no longer want him using the app. I can delete the app, [but] he can also re-download it without it asking me for permission. How can I delete this app from our account altogether and prevent him from re-downloading it without asking permission for that one particular app?

You cannot block an app that has been previously downloaded by a child:

Approve what kids buy with Ask to Buy - https://support.apple.com/HT201089 -

"If a child redownloads a purchase, downloads a shared purchase, installs an update, or uses a redemption code, the family organizer won’t receive a request."
