I’ve been having issues with the phone syncing to the watch. I deleted/hid the app and when I go to the App Store to redownload it my only option is to Open the app. I’ve searched on the phone in Utilities or wherever it may be hiding and I cannot find it. How do I get the icon back to my Home Screen?
I use an iPhone X on OS 14.6.

Restore a built-in app that you deleted
to restore the app.If you have an Apple Watch, restoring an app to your iPhone also restores that app to your Apple Watch.
Try updating to iOS 14.7.1. and to watchOS 7.6.1
Open means you still have it on your iPhone.
What happens when you open it?
FYI: Normally if you try to delete the Watch app from an iPhone that’s paired with an Apple Watch, an alert asks you to unpair your Apple Watch before you can delete the app.
I misread your original advice to search in the library. I was only looking in the categories on the screen. I had never searched there before and didn’t realise I could… This sorted out the issue. Thanks!
Actually, I just found that I can search in the App Library and when I held the icon it let me move it back to the position I had it in. All sorted.

I have found the correct App in the Store. The only option is to Open, not to download.
Try going to the app library (scroll to the last screen) search for it in the search box, if it's there, move it to the previous page.
I did try that. It’s not in any of the categories, not even Utilities. It also doesn’t show up in the search if I swipe to the right on the first screen.
Another thing you can do next time:
Award the poster that Solved (10 points) your question.
Awarding yourself does not gain any points.
