Calls silenced when DND is off

I've read that calls will not be silenced if Do Not Disturb is turned off, regardless of the setting of Silence. With the setting as below I keep missing calls I should be able to answer.

Phone: iPhone 8

iOS: 14.6

Settings:

Do Not Disturb: Off

Scheduled: Off

Silence: When iPhone is Locked

Allow Calls From: Everyone

Repeated Calls: On

Silent Mode: Off

Auto-Lock: Off

What am I missing?
