I’m live in Afghanistan my region is also set in Afghanistan but when I’m going to active or set up Blood Oxygen it give me that it’s not available in your region please me i really need it for myself ?

If you check this, https://www.apple.com/watchos/feature-availability/ you will find that Blood Oxygen feature is not available in your area.
It is based on where you are physically located. You are in Afghanistan, it won't work there.
