I have purchased Apple Watch 6 after checking of ECG app features availability in Cayman Islands . But after activating my watch , while trying to get the app it is showing the region is not in the list to use the said app.
In what country was your watch purchased?
Make sure that both your iPhone and watch are on the most current IOS and OS.
According to Apple branded services the ECG is available in the Cayman Islands

1.Purchased from USA .
2. Both the devices are on updated IOS and OS.
