How do I get my account working again. I had to cancel a card Because of a theft of the information.

Sorry but I don't know of any way to resolve the issue by filling out a complaint form.

I would use the instructions in If you see a message that says 'Your account has been disabled in the App Store and iTunes' - Apple Support

and that may not be easy as lately it frequently results in "We’re sorry, there are no solutions available at the moment. Please try again later."
