How do I enter the four digit code
If I have lost the Apple TV remote, how can I set up my iPhone control center to connect with the Apple TV—when the 4-digit number appears on iPhone, how can I input that?

You would need a physical remote in order to pair with the remote app on an older model. The alternative is to restore it via computer and then pair with a BT keyboard during setup.
