Apple Watch repeatedly disconnects and reconnects to my phone after not using it for about a month

I just started using my Apple Watch again after about a month, only to find out every 5 seconds it disconnects from my phone and takes another 5 to reconnect and this goes on indefinitely - I tried doing the software update but it can't do it because it keeps disconnecting from my phone over and over :| , I have restarted the phone, the watch, the bluetooth, and still nothing works; help please?
