There is no caption or speech bubble feature in iMovie 10.

You can use the Titles feature, such as Reveal Lower Third, to insert a caption on individual video clips. There is no speech bubble feature.

You can create a speech bubble on a transparent background using the Preview app on your Mac. Then you could use the Picture in Picture feature of iMovie to overlay the bubble onto your video.

The speech bubble would look something like this:

You would need to open any random photo in Preview and do an Edit/Select All, Edit/Delete, to create a blank transparency screen. Then do Tools/Annotate/Speech Bubble to create a speech bubble on the transparency screen. Then do Tools/Annotate/Text to insert text into the bubble. Save it and import into iMovie to overlay it onto your video with the Picture in Picture feature.

You can also use the above method to create titles to overlay in iMovie. Create the transparency screen and do Tools/Annotate/Text to create the title in Preview.
