Callers reporting trouble hearing my voice

Every call (inbound or outbound) seems to sound as if I am talking in a tunnel to the recipient. It doesn't matter if I have the iPhone on Speaker, ear or bluetooth as all voice quality is transmitting degraded. I cannot determine if the poor voice quality is the phone or the cellphone signal. How can I troubleshoot the issue to find the source?
