I'm running Catalina on my MAC and OS12.4.4 on my IPhone 5S. My reminders are no longer synched between the two machines and all of the instructions on this issue don't really make any sense to me because, I think, I'm using an old phone which does not handle iOS 13. Can anyone help me here?

Get ready for the new Reminders app - Apple Support
If you want reminders to sync on the old phone then do not upgrade Reminders on the mac.
If you already converted reminders on Mac then I think it is too late because conversion happens in the icloud
Since I have an iPhone 5s, I'm on IOS 12 and not able to covert.
Exactly, so as I said if you have Catalina on your Mac, do not hit the Upgrade option in Reminders.
If you already did then you cannot sync them between Mac and iphone5s
