While fall detection will work without cellular, detecting your fall, to be able to contact Emergency Services, you need to either have the paired iPhone nearby, or have a cellular version of the watch with cellular activated.

If your cellular carrier supports WiFi calling and you have enabled WiFi calling on your iPhone, then you Apple Watch can use WiFi calling as well, so if you are worried about falling at home, that might be an option.

And my experience with Bluetooth range is iPhone and Apple Watch are somewhere in the same 3-4 bedroom house. My Wife frequently answers the phone on her Apple Watch from the 2nd floor bedroom when her iPhone is in the first floor kitchen on the opposite side of the house. 50-60 feet, some 2x4 drywall covered walls and one floor to traverse.
