The App Store app is not showing up on my gen 4 Apple TV. We have done the restore and it still does not show up.

Apple TV 4, tvOS 10

The App Store app cannot be removed, but can be made inaccessible through Restrictions.

The App Store app icon can have been moved (accidentally) to an unexpected location (e.g. in a folder).

Restrictions is turned off and there are no folders on it.
