IOS 13.1.2 Silence: Only has two options "Always" and "When the phone is locked", How do I shut the silence off so that I may receive calls when phone is locked?

Those options only apply when Do Not Disturb is on. If you don't have Do Not Disturb turned on it doesn't matter whether you have that option set to Always or When iPhone is Locked.
