Missing health app data after upgrade to iOS 13

Using an iPhone SE.

Prior to iOS 13, there was functionality to add data in the notes section of the Health App. It was in this section , I recorded medications, surgery and dates, allergies.

Now after this upgrade to iOS13, that section of data is unexpectedly missing and this past year or two several details were added on my phone that was not listed elsewhere.

How can I get the missing data from the original pre-upgrade version of the Health app?
