How to erase a device in someone else's possession

My old employer has my work iPad that was connected to my Apple ID. She wants to give the IPad to someone else, but it keeps asking for my Apple ID, even though I think I correctly erased the device from my account (I am not seeing the iPad listed on any of my other Apple devices).

How can she get into the iPad without me giving her my personal Apple ID password?

If its still tied to your iCloud Account, you should be able to log in to iCloud.com or the Find my iPhone app on another device with your account and use the find My iPhone app to locate and erase it.

If it does not appear there, nor in the devices list on another device that is using the same Apple ID for iCloud, then its been successfully removed from your account, and your password is not needed. Something else would be wrong then.

What steps did you follow exactly to erase the device?
