do I have to pay extra for the apple watch witch cellular?

Yes you do for both the watch and a cellular plan.
Make sure you are in a country which support cellular apple watches.

You can NOT sign up for cellular, and use the Apple Watch as if it were a GPS only model.
As long as your iPhone is nearby, the Apple Watch will do everything th cellular model will do.
I should also say that if you buy a GPS only model, your iPhone cellular carrier does not know and does not care.
Thus the there is no extra fee for a GPS only model, nor for a cellular model if you do not sign up for Apple Watch cellular service.
