iOS 26 content restrictions not preventing app downloads

Hi I am running iOS 26.

and the phone is setup to not apps to be downloaded by the users per the content settings however recently users are being able download apps.

by searching for the apps and using hitting Get.

Also, the phone allowing users to reset the passwords for screen time.

I set it up so I did not set up a recovery password. Did apple change something?

The issue with the password looks like this

It should not have a email listed with the pin.
