I was sent two similar emails from iOS Mail app (for an online order return) each with unique QR codes as attachments. The emails I received in my own iOS Mail had only the first of the QR attachments , ie the separate emails had both ended up with the *same* attachment.

The Mail app reused a cached QR image because the two emails had similar attachment metadata. The actual QR codes on the server are still different. Forwarding the emails, viewing them on another device, saving the attachments, or resetting Mail’s cache will reveal the correct, unique QR codes.
