Got into the loop of ‘ Manually Reformatted Disk Detected’ Any help here?

  • Format the Macintosh HD volume
  • reinstall mac (mac os 26.0.1)
  • after/while installation- seeing the popup asking to restore the security policies
  • - Tried formatting complete disk and
  • Tried using external disk formatted as guid(apfs) and tried reinstalling the mac os this - same issue

You can try a DFU Firmware Revive, but I'm guessing you will need to go with the nuclear option of a DFU Firmware Restore. Unfortunately this option requires access to another Mac running macOS 26 Tahoe......macOS 15.x Sequoia may also be an option as well, not sure about the latter though

That error usually means the internal SSD’s partition map got misconfigured during formatting or macOS isn’t detecting the correct APFS container.


Here’s what I’d do: boot into Recovery (hold power until “Loading startup options”), open Disk Utility, choose View > Show All Devices, and erase the entire internal drive (not just “Macintosh HD”) as APFS (GUID Partition Map). Then reinstall macOS directly from Recovery without connecting any external drive.


If it still loops, reset NVRAM and run Apple Diagnostics, it’ll confirm if there’s a firmware or hardware issue with the M4’s internal storage.
