Why are my tones super quiet or not sounding when my volume is max, mute OFF, and DND OFF?

I have a 16 pro max on iOS 18.1.1

I’ve noticed I keep getting missed calls and not hearing any ringtones or vibrations from my phone. It doesn’t always happen but enough times that my hubby has started pinging my phone through the find my app just so I’ll know he’s trying to reach me.

Sometimes I have low service so I thought that was the issue. However, I’ve noticed I’m not hearing my alarms or timers all the time either. Yesterday I had a 20min timer set and took a nap. Barely heard the sound as if it was muffled under a pillow. Checked everything and my volume was at max, mute OFF, DND OFF. Decided to check again and set timer for 10 seconds, turned screen off, set phone down. Same super quiet tone that hubby couldn’t even hear 4 feet away. Why is my phone only allowing SOME of my tones to come through loud while most are quiet or muted? Calls, alarms, timers, messages, possibly all tones. Been going on most of the year. I’ve done 1-2 updates since I noticed the issue.
