2017 iMac with macOS Ventura 13.7.6 is slow, fan on max and kernel_task consuming 540% of CPU

Did Software Update from Monterey to Ventura about 2 months ago. Now on v13.7.6.

Fan constantly running full speed, everything very slow including keyboard input, no apps open except Finder and Activity Monitor which shows 525%-565% CPU for the Process Name: kernel_task. Everything else shows low single digits, most of which are 0%. How to fix this?

Actions to date:

  1. Restart - same outcome
  2. Shut down, wait 2 minutes, Start up - same outcome
  3. Run “D” diagnostics - shows PFM006 (SMC issue)
  4. Reset SMC (shut down, pulled plug, left for 15 seconds, plugged in, waited 5 seconds, started up - same outcome

Any help appreciated.
