I want to create a playlist that includes all my "Loved" (starred) songs and then add in all of my favorite songs that I haven't heard in the past x months. This is what I've come up with, but I have to think I'm missing something.

You need nested rules to do what you say. Make a set with "match any" to pick up all Loved songs and 5 star songs. Then nest it with your Last Played rule as you have, but I think you want "is not in the last" rather than "is in the last."

I need to add, that I want all of my "Loved" songs and then add in songs from the "Favorites". This is my favorites playlist rules.

If you don't like (or don't trust) nesting, you can do it with 2 playlists. Keep the one you defined in your first post. Then add a second smart playlist with rules as follows:
