recently I got brand new IPhone 16 pro now I need personal hotspot but it is grayed out from menu.

Persona Hotspot will be grayed out if Cellular Data in not enabled. You do have Cellular Data enabled on your phone, correct?
Hotspot is a carrier feature. Your iPhone displays the settings that your carrier is providing.
Check with your carrier again. Hotspot is an optional feature with some carriers, available for an extra charge.

As I said my provider has no restrictions.
The hotspot feature was available on my previous 12 pro max. The carrier and the plan is the same.
resolutions easy: APN needs to be filled. Apple missed that recommendation in their guidelines, but recommended to reset network settings (I don’t know if it clears APN).
Are you using a supported carrier? Personal hotspot has to be explicitly provisioned on the line for an iPhone. If you're using an unsupported carrier, that may not even be possible.
So you're using an unsupported carrier. If you were using a supported carrier, you would not need to manually enter the APN information.
You will have to work this out with your cellular carrier.
