Activating Sleep Mode in the Control Center of the watch will not help to track sleep in the Health App.

Sleep Mode will only make sure, that your settings for not being disturbed during sleep, for example, are active.

You will have to set up a Sleep Schedule.

Did you already try to force restart your iPhone and Apple Watch after setting up your Sleep Schedule?

Force restart iPhone - Apple Support

To force restart your watch, press and hold the Digital Crown and the Side button together for about 10sec, until the Apple logo comes back again.

Does your iPhone and Apple Watch enter Sleep Mode according to the Schedule and show the Sleep Focus icon?
