Perhaps they’re just referring to the “Start Listening”, “Stop Listening” features of Voice Control. Which is technically it turning on and off. But not completely, as that has to be done in Settings.
I know you know this already, I’m just musing. 😆
I mean there’s a few ways. Siri could turn it on I’m sure, you could accidentally turn it on with Vocal Sortcuts, if you have it set up. (Settings > Accessibility > Vocal Shortcuts. Triple clicking the side button to turn it on would be intentional so that’s off the list.
I’m not sure if you could accidentally turn it on with Shortcuts or not. I haven’t fiddled with them yet.
Could eye tracking turn it on somehow? I don’t know I haven’t fiddled with that either.
Just try turning it off in Settings for now, and let us know if the issue persists.
I haven’t gotten around to setting up Vocal Shortcuts yet, so that can’t be it. AHA! Three side clicks DOES turn it on and off—didn’t realize that was a thing. I must have done that inadvertently each time it happened (how I don’t know yet, but it’s obviously happened more than once). Where is that feature controlled? Now that I know what’s doing it, I can avoid it, but it would be nice to know.

In 17 years I have never known ANY Accessibility item to go on by itself.

You’re right! That feature is located at: Settings > Accessibility > Accessibility Shortcut.
Now scroll down, and deselect “Voice Control”.

Hm. Is there some way one can inadvertently switch it on without going into Settings? The visible sign that it’s on is a green symbol up in the top right corner of the phone (not the little orange dot that comes on in notch). The aggravating sign that it’s come on is that while you are trying to text someone in a public place, your text gets overrun by transcriptions of everything everyone around you is saying...

Yeah. That will all go way once you turn it off in Settings.

Go to: Settings > Accessibility > Voice Control and turn it off as depicted below. This should prevent it from turning on again.
Do you mean VoiceOver or Voice Control? Neither will turn on automatically; someone must turn them on in Settings/Accessibility.
