How to add Frequently Visited Icons to Start Page

Yesterday, I tried to delete one single icon, and when I right-clicked it, and hit Delete, ALL of them Vanished!

Great, so it's been a day, and even though I have that option checked in the Start page settings window (lower right-hand corner), not a single one has been created, and I have many days of my search history, so I don't know why it can't simply add the most frequent ones.

And also, HOW CAN I ADD a site to either the Frequently Used, or the Reading List?? I can't believe it's this much an enigma, and not simply as straightforward as pretty much every other browser is.
