Apps being removed from my Home Screen on iPhone 16 Pro Max

I’ve being consistently seeing my apps being removed from my Home Screen.


I have 3 different focus modes set up on a timer to increase my productivity during the day but whenever I switch between focus modes, I am seeing certain apps disappear off the Home Screen.


I am having to add these apps back each time my phone switches focus which is very annoying and time consuming, which is now decreasing my productivity.


I found a Reddit post which had a workaround that involved pulling apps across from my App Library rather than searching for the apps with smart search but it hasn’t solved anything and I’m still seeing apps disappear.


I’ve added a photo below showing the gaps I am being faced with.

I am running iOS 18.1.1 on an iPhone 16 Pro Max

This is still happening for me too. iPhone 15 PM iOS 18.3. I’ll try your advice and see if that makes a difference. I’ll report back if it works.

This happens to me every day and it's so annoying. Every night when my phone switches to sleep mode, and then personal, then work mode, a couple apps disappear from my homescreen. I have to re-add them back to each focus mode again. I can trigger it by switching focus modes manually, but sometimes it doesn't always remove the app. But it does seem to happen everyday or every other day. It's only been an issue on this latest major iOS 18 update.


PS. SravanKrA's answer is irrelevant to what OP and I are discussing.
