I am currently using iphone 15 pro. I am trying to access a banking app called “YONO SBI” but every time I open the app I get a warning like this
I cannot figure out if my phone has really been hacked or if there’s a problem with the app

solution is Just update the app, this will be resolved. May be new password app is activated.

How to resolve it
You wait for the developer of Yono to update their software, this is not an Apple issue.
If you did not jailbreak your phone, then there is a problem with the app. Contact the app developer for help.
How I do if my mobile is jailbroken.
if you jailbroke your phone, you could try to erase it and set it up as new. Beyond that, we can’t help you.
