Multiple Family Group being formed

Whenever my daughter requests an in-app purchase, a download, or more screen time, a new group gets created with me and my partner, instead of using the existing group. My partner is listed as her parent/guardian, and I’m the organizer. As a result, I now have about five groups with the same two people.

How can I prevent a new group from forming each time
