Apple Watch 9 w/ version 10.5
When texting within a group chat, if someone sends a photo the watch splits the texts into two separate groups. Everything on phone is still in same group message.
I've tried re-pairing, erasing, restarting,…everything in the book to try to solve the problem but nothing works.
Is this an Apple issue or service provider issue?

Same exact thing has started happening to me as well. Messages are "normal" and grouped on iphone 13 pro max with ios 17.5.1, and Apple Watch Ultra with watch os 10.6.1. But on apple watch it is splitting out replies into separate texts.
Looks like it is an Apple issue. Tried un pairing and re pairing watch, reset watch, and still no change. It is still splitting group MMS and adding me as a contact. For example a group mms has 12 participants - when it "splits" the message it adds me as the 13th recipient.

I have been having the same exact issue you described with my Apple Watch Ultra and my iPhone 12. This issue started happening maybe 6 months ago and was fine for a year and a half prior .
It’s super annoying , I’m hoping Apple can fix this.
