You might want to check your iPhone has the most recent iOS update:
About iOS 16 Updates - Apple Support says iOS 16.4.1 is the newest.
If you are already using iOS 16.4.1
You might want to try re-starting your iPhone to see if that helps restore order to the clock app: Restart your iPhone - Apple Support
If still not able to set alarms:
