I am the family organizer. I used to have an iPhone which I got rid of and now have a Samsung.
I still have my Apple Watch Ultra which my wife is trying to setup for me, but I am not showing in the list of family members when she goes through the "Setup for Family Member" process.
Is there a particular workflow for this or because I am the organizer nobody can setup a watch for me?

To be clear, you cannot be the family organizer, if you don't have an iPhone. But your wife can set up an Apple Watch for you to her iPhone, using the directions here --> Set up a family member’s Apple Watch - Apple Support (IN)
Tragically these steps do not work as I do not show up in their list of Family Members.
However this is my family shown on my MacBook.
Perhaps there is a setting that I changed at some point in Family Sharing that is preventing me from having a watch setup by the other Parent/Guardian.
You're trying to use Family Sharing, which isn't possible since you don't have an iPhone. The only way you can have an Apple Watch without an iPhone is to own a cellular Apple Watch and use the Family Setup link I provided.
If that isn't clear or isn't working, you'll need to contact Apple Support for more assistance.
As stated in the original post I have an Apple Watch Ultra which has cellular and has an active AT&T plan.
I have an iPhone which is attached to my account which I use for work and on another cellular account. This should be irrelevant in this scenario.
I haven't be limited in any other capacity regarding my Apple Products because I move between iPhone and Samsung devices every other year for the most part.
I already contacted Apple Support who provides the same link with steps which do not work.
So I am posting here with hopes someone actually knows what could be causing the behavior of my account not showing for the other adult.
If you have a work iPhone and your Apple Watch is paired to the iPhone, the Apple Watch and iPhone MUST be on the same account. You can't just switch back and forth between an iPhone and Samsung with an Apple Watch. Otherwise, I can't offer any more help.
