Yes, you can acquire an ECG.
There are various sorts of pacemakers.
Some are active at all times, others are monitoring and only pacing as needed.
Monitoring an ECG with Watch should not adversely effect a pacemaker.
The pacemaker may or will effect the heart rate.
You will, however, want to first discuss this with your cardiologist. Your cardiologist will know whether Lead I monitoring—what Watch provides—would be useful here, and will know if there may be or are other medical issues or concerns here.
