I'm using the shifting tiles.
None of the photo-based savers seem to work across both displays. They used to before Ventura.

What external display and how is it connected to the iMac?
Try Apple > System Setting Desktop & Dock > uncheck the boxes for Spaces in Mission Control.
Then use Safe Mode followed by a normal restart and test the screensaver.
see > How to use safe mode on your Mac - Apple Support
If that doesn't help, contact Apple Support and/or send Feedback to Apple.
What screen saver option are you using?
Some of the native macOS screen saver options work on dual displays and some do not.
I have couple live screen savers from the App Store, that actually span across dual displays.
Hi, I've had this problem since 13.3 and it is not fixed in 13.3.1 :-(
I've just spoken with Apple Support. They confirm this is a known issue that is being investigated.
Thanks! :) 👍
