Internal Screen Damage without External Screen Damage

Can someone tell me how can be internal screen damaged without any damage on external screen and case? (even a any dent or scrath)

They ve said me on apple service "This may happen"

1)If its because of small pressure (like a finger or apple pen) which cannot crack the external screen its a production fault

2)If its because a big pressure according to physics rules outer screen must be cracked as well

Mine is first but apple thinks its a user fault.... I am waiting your opinions
