Apple ID reveals true name. How can I get a screen name for Apple communities?
A member with reporting privileges tells the moderators that you are seeking a change. I have done that on your behalf.

Nope, it's forum-wide.
The moderators will contact you by email, typically the one you use to sign in here. Be on the lookout for it.
