Why is my iPhone 12 Pro Max upgrading to iOS 15.7 and not the iOS 16.0.3?

Go to Settings->General->VPN & Device Management. If you have Beta profile delete it.

Otherwise scroll down to below the iOS 15.7 updater and there should be an option for iOS 16 there.
